(d) A person providing a general power of attorney shall designate the person who shall receive notice of the general power of attorney or power of attorney of the person providing the power of attorney. A person providing an actual power of attorney shall specify in the power of attorney that the person providing the power of attorney will receive written notification of that actual power of attorney. An application for an actual power of attorney must be accompanied by a copy of the person's passport and visa, unless the passport or visa is invalid, the applicant is a refugee, or the applicant holds a permanent residence visa as of the effective date of the power of attorney. Any applicant who has a permanent resident visa but who did not provide an official copy of that visa to the Department of State upon becoming a permanent resident of the United States shall include a copy of said visa in the application filed with Customs.
